Advanced Practice Provider - Nurse Practitioner
About the role
The Advanced Practice Provider (APP) plays a pivotal role in coordinating the continuum of care and managing the overall patient experience within the USC Brain Health Network (UBHN), ensuring good communication, efficient and patient-centered operations, and effective service delivery for every patient. They are responsible for coordinating and navigating the care for all patients referred to the UBHN for services between the primary care provider and referrals to ancillary services and specialties.
Responsibilities
- Serves as the primary point of contact for all primary care patient referrals within the UBHN, taking calls from providers/offices and connecting with patients.
- Works with referred patients to ensure that all clinical services are received timely and appropriately, and results are communicated within the UBHN following established protocols.
- Follows up with patients to provide appointment reminders and to ensure patient comprehension and compliance.
- Works collaboratively with assigned physician(s) to regularly discuss care and management of assigned patients.
- Accept and perform other related duties as assigned.
Requirements
- Requires a bachelor’s degree in a job-related field and 3 or more years of job-related experience, which may be substituted by an equivalent combination of job-related certification, training, education, and/or experience.
- Current licensure as Nurse Practitioner with SC LLR, in good standing.
Qualifications
- Ability to communicate effectively.
- Ability to assess, identify, and address individual needs.
- Knowledge of or the ability to learn medical terminology, use medical equipment and follow clinical orders.
Skills
- Preferred at least one year of experience working in a physician practice.
- Preferred experience coordinating care of patients across different areas/services of a health system.

Pay
To be determined based on experience and qualifications.
Schedule
Standard working schedule: 8:30am – 5:00pm Must be willing to work a flexible schedule to meet the needs of the department.
