Key Job Responsibilities
- Takes care of terminally ill patients at home.
- Operates in a systematic manner to provide comprehensive healthcare for terminally ill patients.
- Assesses and reassesses the nursing needs of the Hospice patient.
- Performs Face to Face visits timely.
- Provides dietary counseling.
- Provides Hospice nursing services, treatments, and preventive procedures.
- Initiates nursing procedures appropriate for the patient’s Hospice care and safety.
- Observes signs and symptoms and reports any unexpected changes to the physician and IDG members.
- Teaches, supervises, and counsels the Hospice patient and family members about providing care for the patient.
- Supervises and trains other nursing service personnel.
- Attends team conferences.
- Maintains records as required by Hospice.
- Follows the policies and procedures of Hospice.
- Communicates to the supervisor if unable to meet a patient’s need or perform a procedure.
- Participates in the on-call system and provides on-call coverage when unavailable for assigned duties.
- Maintains skills and knowledge.
- Works with interdisciplinary group concept of patient care.
- Collaborates with other professionals to implement the plan of care.
- Coordinates the implementation of the plan of care for patients residing in SNF, NF, ICF, or MR.
- Organizes work schedule and utilizes time management to attend all required meetings.
- Complies with agency infection control policies and protocols.
- Affords assistance with orientation, teaching, and training as requested.
- Completes clinical notes for each service provided.
Qualifications
- Regularly assesses and reassesses the nursing needs of the Hospice patient.

Pay Rate
$130.00 per visit
Regulatory Requirements
- Licensed or registered in which he/she currently practices.
- Passes a criminal background check & MVR check.
- Completed health statement acknowledging ability to perform the duties of the position.
- Valid state driver’s license.
- Proof of current automobile insurance.
- Current CPR card; Online CPR certification accepted; Hands-on CPR certification preferred.
- TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).
Industry Experience
- Ability to work independently, make accurate, and at times, quick judgments.
- Ability to supervise others appropriately.
- Ability to respond appropriately to crisis outside of a hospital setting.
- Acceptance of and adaptability to different social, racial, cultural and religious modes.
Education
- Graduate from an accredited school of nursing.
- Currently licensed as a Registered Nurse and Nurse Practitioner to practice in the state.
