About the role

Spurwink is seeking a Registered Nurse (RN) with Maine licensure to join an interdisciplinary team serving adults and children with various mental health needs, including serious mental illness and substance use disorders. This position involves providing nursing consultation, interpreting health data, and integrating health and wellness goals into treatment plans. Duties include conducting home visits, facilitating group interventions, and collaborating with multidisciplinary teams.

Responsibilities

  • Engage, assess, monitor, educate, coordinate, and provide interventions for participants and families to improve quality of care and health outcomes.
  • Teach and educate participants and families about health and wellness, safety factors, self-management skills, medications, self-monitoring, action plans, long-term planning, and accessing care.
  • Conduct in-person visits with participants in the community, at home, hospitals, care facilities, physician offices, based on participants' needs.
  • Facilitate group treatment and education interventions.
  • Provide health information related to participant medical history and needs, medications, co-morbidities, immunizations, dietary needs, and the behavioral, social, psychological, and environmental determinants of health.
  • Assist the treatment team with developing, implementing, and monitoring Individual Service Plans (ISPs), and document ongoing nursing care and interventions in the electronic health record.
  • Maintain regular contact, effectively collaborate with, and facilitate referrals to community providers to advocate for and improve the care of participants.
  • Adhere to procedures and practices outlined in Health Home MOUs.
  • Maintain required records and documentation, collect, track, and submit participant-related data to support evaluation of outcomes, program goals, and performance.
  • Provide resources for staff, colleagues, and external members of a participant's care team.
  • Support the Medical Assistant by coordinating sample deliveries from drug reps, enrolling/re-enrolling patients in patient assistance programs, administering antipsychotic injections and oral medications, packing medication caddies, and assisting with appointment scheduling.

Requirements

  • Maine licensure as a Registered Nurse (RN).
  • Three years of post-licensure experience in community health nursing, family nursing, pediatric nursing, hospital, or psychiatric nursing preferred.
  • Direct service experience working with individuals with severe and persistent mental illness, serious emotional disturbance, and developmental disabilities is preferred.
  • The ability to exercise professional judgment in making decisions, communicate effectively with service recipients, family members, case managers, administrators, community providers, and other selected personnel.
  • A high level of organizational and time management skills.
  • Computer literacy and proficiency in navigating an electronic medical health record, utilizing HealthInfoNet, generating PowerPoint presentations, and presenting basic data summaries to management.
  • Knowledge of psychosocial, mental health, and vocational assessments, counseling theories such as MI, CBT, and DBT, and the physical and psychological effects of mental illness and substance abuse is preferred.

Qualifications

  • Minimum educational requirement: Maine licensure as a Registered Nurse (RN).
  • Minimum experience requirement: Three years of post-licensure experience in community health nursing, family nursing, pediatric nursing, hospital, or psychiatric nursing preferred.
