About the role
The Patient Navigator assists patients and families by coordinating care, providing education, emotional support, and ensuring smooth transitions post-discharge. They collaborate with healthcare staff to develop individualized care plans, conduct assessments, and facilitate communication among providers to promote positive patient outcomes and prevent readmissions.
Responsibilities
- Develop and evaluate personalized care plans based on patient needs and goals.
- Assist with patient throughput, discharge planning, and referrals.
- Serve as a clinical resource within the interdisciplinary team.
- Communicate critical risk information to ensure patient safety.
- Perform physiologic and psychosocial assessments.
- Adapt education and care strategies to individual patient needs, including cultural and ethical considerations.
- Advocate for patient rights and evaluate outcomes for care revisions.
- Document interventions and coordinate with team members.
- Collaborate with community resources and provide staff education on geriatric needs.
Requirements
- Minimum of 3 years in an acute clinical setting working with relevant populations.
- Bachelor's degree in nursing.
- Proficiency in English, communication, and computer skills.
- Current Louisiana RN license and BLS certification.
