Nurse Navigator - Full time, Days - Cardiology - Atlantic Health Chilton Medical Center
About the role
The Nurse Navigator is responsible for enhancing communication amongst the patient/family, the interdisciplinary team and all interdepartmental needs. Assists in the development of strategies to reduce LOS and re-admissions. Part of the cardiovascular team which monitors LOS and re-admission data for CHFAMI, COPD, and pneumonia. Monitors care process and develops approach to improve care efficiency and patient outcomes. Performs other related duties as assigned.
Qualifications
- Education: Bachelor's/Master's in Nursing or related field
- License/Certification: Current NJ (New Jersey) Registered Nurse License required, BLS required
- Relevant Experience: 3 years of cardiology nursing, Quality background preferred
