About the role
The Nurse Navigator (NN) acts as a multidisciplinary team leader and liaison between patients, family, physicians, clinical care teams, community organizations and BHSF leadership. The NN is responsible for leading coordination of care across the continuum, providing education, support and resources to patient/family, facilitating shared decision making, patient advocacy, ensuring timely access to quality multidisciplinary care and patient satisfaction. The NN will monitor individuals, populations and program outcomes and engage in quality improvement activities.

Requirements
- Bachelor of Science in Nursing (BSN)
- Active Registered Nurse license
- Certification in area of expertise
- Advanced Cardiovascular Life Support (ACLS) certification
- Basic Life Support (BLS) certification
- Five years of nursing experience in key relevant areas strongly preferred: MedSurg, Oncology Care (Inpatient/Outpatient Oncology, Infusion, Radiation, BMT, etc.), Patient Navigation or related service (Case Management, Care Coordination, Social Work, Population Health, etc.), or specific Disease Sites (Breast, GI, GU, Neuro, etc.)
- Experience working in a hospital, managed care, or medical office preferred
- Proficiency in Microsoft Office, EMR applications (Word, Excel, SharePoint, TEAMS, HEDIS, STAR, etc.), and typing
- Bilingual English/Spanish preferred
Pay
Estimated salary range for this position is $87,755.20 - $116,714.42 per year depending on experience.
