About the Program

The San Julian Oasis (SJO) Recuperative Care Program is an interim housing program that provides bed space for eligible patients. To qualify, patients must be unhoused, have an acute medical condition, and be medically and physically stable. This respite program offers medical care, case management, personal care, three meals per day, access to long-term housing resources and placement, and reunification services for clients who have lost contact with their family or support network.

Principal Responsibilities

• Conduct nursing assessments for new admissions and oversee patient screenings.
• Assist medical providers with procedures and ensure proper execution of treatment plans.
• Monitor medication management processes, supporting clients in self-administration and ensuring medication safety.
• Organize and maintain medication inventory, ensuring proper disposal of expired medications in coordination with LVNs.
• Ensure timely ordering and stocking of medical supplies.
• Provide patient education, facilitate specialist referrals, and coordinate hospital appointments as necessary.
• Ensure adherence to HIPAA regulations in handling protected health information (PHI).
• Support program leadership in meeting all outcome measures established by the Department of Health Services
