Job Summary
HealthRIGHT 360 is a nonprofit organization dedicated to providing comprehensive healthcare services to vulnerable populations. We integrate physical and behavioral health to empower individuals to overcome challenges and promote health equity. The Integrated Care Center clinic offers a wide range of services including primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We are seeking a mission-driven service leader with experience in managing and supporting staff.
Key Responsibilities
Provide nursing care as part of an integrated care team, document care promptly, and follow charting protocols.
Facilitate team functioning and efficiency by taking responsibility for additional patient assessments or medical assistant functions.
Assess and provide nursing advice to patients, respond to patient requests, and inform clients of lab results.
Prioritize and take responsibility for patients with complex medical issues, and follow up with patients regarding recent ER visits and inpatient stays.
Work collaboratively with mental health staff and other ICC services to facilitate integration between primary care, mental health, and other ancillary services.
Manage a team of nurses to deliver high-quality results, provide ongoing recognition, feedback, and support, and develop nurses' professional skills.
Participate in training and assist in continuous quality improvement through on-going problems/needs identification and problem solving.
Maintain quality assurance logs and ensure the quality of medications, supplies, medical equipment, and laboratory testing.
Qualifications
Graduation from an accredited school of nursing with a Bachelor's of Nursing.
Registered Nurse License by the California Department of Consumer Affairs (DCA).
Valid California license as a registered nurse.
BCLS/CPR certification.
A minimum of 3 years of managerial/supervisory experience.
Experience with electronic medical records.
Two years of direct patient care experience and one year in ambulatory care and/or a community health center.
Experience working with populations with varying lifestyles, ages, sexual orientations, ethnic and cultural backgrounds, and economic status.
Two or more years of leadership experience in a community healthcare setting; preferred experience in leading other RNs in a multidisciplinary setting.
Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency.
LGBT Cultural competence and ability to work with a diverse population.
Strong proficiency with Microsoft Office applications, specifically Word, Outlook, and internet applications.
Knowledge of co-occurring disorders and trauma-informed treatment.
Strong phlebotomy skills.
Comfort working on teams and with safety net populations.
