What You’ll Do:
- Serve as the primary clinical coordinator for residents transitioning into and out of the the Long Term Care or Transitional Care Unit
- Oversee and manage clinical care plans to ensure optimal recovery and successful discharge outcomes
- Collaborate with physicians, therapy, case management, social services, and interdisciplinary team members
- Monitor resident progress and proactively address changes in condition to prevent rehospitalizations
- Educate residents, families, and staff on care plans, expectations, and recovery goals
- Support admissions, assessments, and care coordination for short-term rehabilitation residents
- Ensure compliance with Federal, State, and Local regulations, as well as organizational policies
- Participate in weekend on-call rotation to support clinical operations and continuity of care
What You’ll Need:
- Current Illinois LPN or RN license required
- 2+ years of nursing experience in skilled nursing, transitional care, rehabilitation, or long-term care preferred
- Prior leadership, care coordination, or nurse manager experience preferred
- Strong clinical assessment and critical thinking skills
- Experience working with interdisciplinary teams and managing complex care transitions
- Able to work independently and manage multiple priorities in a fast-paced environment
- Commitment to providing compassionate, resident-centered care
