Compensation range: $90,000 - $100,000 annually.

About the role

The Nurse Manager is a licensed professional nurse responsible for coordinating nursing services to patients enrolled in Health Services. The Nurse Manager facilitates quality services by providing primary care to patients, including assessment, treatment, care planning, and nursing care evaluation. The role also includes staff training, performance evaluations, medical and supply inventory management, patient panel management, and clinic patient flow. Additionally, the Nurse Manager provides backup support to the primary care and mental health care teams as needed.

Responsibilities

• Facilitates health center huddles.
• Manages day-to-day health center clinical flow and operations.
• Performs nursing triaging and assessment.
• Participates in the training of members of the patient care team.
• Evaluates performance of the Medical Assistant and Registered Nurse.
• Ensures nursing practices of the care team and own practices comply with nursing standards, local, state, and federal guidelines, and organizational policy.
• Ensures compliance with infection control, safety, and confidentiality guidelines (OSHA, CLIA, HIPAA, etc.).
• Manages the patient’s Plan of Care by supporting patient transition through continuity of care (e.g., hospital to home via outreach, assessment, and referrals). Assists in the timeliness of doctor follow-up.
• Reviews work to be completed, develops plans based on patient population needs, skill levels of nursing care team members, and PSI protocols, policies, and procedures.
• Serves as clinic liaison to pharmacy and laboratory vendors.
• Interfaces with physicians and all staff members related to the patient care team; provides backup support as appropriate.
• Coordinates phone calls from patients regarding questions and refers to appropriate resources (e.g., physician, case management).
• Provides patient education by supporting and teaching patients about self-care management goals and activities.
• Coordinates medication administration and reconciliation for the co-located mental health clinic.
• Calls patients at the physician’s request to ensure compliance with treatment plans and/or response to prescribed treatment.
• Follows up on abnormal laboratory results requiring patient action, including altering medication dosage as per physician’s order or accepted protocols, and dietary changes.
• Coordinates requests for medication refills according to Housing Works procedures and protocols.
• Coordinates outpatient work-up and treatment plans when required (e.g., further clinical testing, x-rays, and facilitates communication between physicians and healthcare providers).
• Provides direct care for patients, including vital signs, blood pressure monitoring, and medication administration.
• Monitors patients during medical emergencies while waiting for 911 assistance and transport to the Emergency Department.
• Utilizes EMR to document all patient care and medication administration accurately and timely, in compliance with PSI policies and procedures.
• Promotes patient education through assessment; develops strategies to maintain high levels of wellness for at-risk populations.
• Maintains licensure and certification as required and is responsible for own professional development through periodic attendance at ongoing educational events.
• Participates in program evaluation and quality improvement activities.
• Supports implementation of programs and procedures developed at an organizational level.
• Understands and supports the practice of other team members and refers patients appropriately.
• Assists with compliance with FQHC standards and regulations where applicable.
• Performs other duties as requested by the supervisor.

Requirements

• Bachelor’s Degree in Nursing from an accredited nursing program.
• Current registration and active license as a New York State Registered Nurse.
• Experience as a nursing care practitioner.
• CPR certification or completion of certification during employment.
• Fluency in written and spoken English.
• Demonstrated understanding of the needs of clients in treatment for HIV/AIDS and substance abuse.
• Ability to see, hear, and speak to clients.
• Bilingual English/Spanish preferred.
