Duties and Responsibilities

  • Lead & Inspire
    • Provide leadership, mentorship, and support to a multidisciplinary team of Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Medical Assistants (MAs).
    • Foster a culture of collaboration, accountability, engagement, and continuous learning.
    • Coach, develop, and empower team members to achieve their professional goals and deliver exceptional patient care.
    • Oversee day-to-day clinical operations, including patient flow, staffing, scheduling, and clinical support functions.
    • Ensure compliance with infection control standards, quality measures, Joint Commission requirements, and UDS performance indicators.
    • Monitor clinical performance and implement best practices to enhance patient outcomes and operational efficiency.
    • Lead quality improvement initiatives utilizing methodologies such as PDSA, Lean, and evidence-based practices.
    • Analyze performance metrics and identify opportunities to improve care delivery and patient satisfaction.
    • Participate in organizational committees, strategic projects, and cross-functional initiatives that advance FHCHC's mission.
    • Collaborate closely with providers and interdisciplinary teams to deliver integrated, coordinated care.
    • Provide direct patient care when needed, including assessments, medication administration, documentation, and care coordination.
    • Ensure every patient receives compassionate, respectful, and culturally responsive care.

Qualifications

  • Active Connecticut RN license required; BSN preferred
  • CPR certification from the American Heart Association required
  • Minimum 2 years of ambulatory care experience required; FQHC experience preferred
  • At least 1 year of supervisory or leadership experience in an ambulatory setting
  • Strong leadership, communication, critical thinking, and team-building skills
  • Experience with quality improvement methodologies such as PDSA or Lean preferred
  • Proficiency with EHR systems, Microsoft Office, and web-based tools
  • Bilingual English/Spanish preferred
