About the role
The Alfred Health HITH unit is a leading provider of acute home based health care in Victoria. This dynamic role involves collaborating with multidisciplinary teams across a wide range of medical and surgical specialities within Alfred Health, and with outsourced service providers. Patients access HITH for a range of treatments including for Cystic Fibrosis, Transplant, Heart Failure, Burns, and Cancer.
Responsibilities
- Collaborate with multidisciplinary teams across a wide range of medical and surgical specialities within Alfred Health, and with outsourced service providers.
- Treat in excess of 50 patients daily.
- Provide care autonomously in the community.
- Work with patients, their families, and establish relationships to achieve the best possible outcomes.
- Display core nursing values of patient centred care, trust, and respect in every interaction.
Requirements
- Registered as a Registered Nurse with the Nursing and Midwifery Board of Australia via AHPRAD.
- Demonstrated interpersonal skills.
- Demonstrated organisational skills.
- Demonstrated motivation and enthusiasm.
- Demonstrated team player.
- Experience within the community nursing setting desirable.
Qualifications
- A current Victorian Driver's Licence.
Skills
- Recent acute care experience.
- Advanced clinical assessment skills.
- Motivated, flexible, and strong communication skills.
- Commitment to care beyond the hospital walls.

Fixed term role until January 2027. Part time (up to 72 hours per fortnight).
