Nurse Case Manager/UKHC

Department: Continuum of Care | Location: Lexington, KY | Full-time staff position

About the role

The RN Case Manager facilitates care coordination for patients and families when clinical issues affect effective use of inpatient or outpatient services. Responsibilities include organizing tests and results to prepare for discharge, assisting with discharge planning, liaising with departments and agencies for continuity of care, and recommending evidence-based practices to meet patient needs. The role also involves arranging home health, durable medical equipment, or infusion therapy, completing referrals, providing case management education, collaborating with Social Work Case Managers for high-risk patients, and participating in departmental coverage.

Responsibilities

• Coordinate care for patients and families to address clinical challenges impacting treatment utilization.
• Organize and facilitate access to tests, procedures, and results to prepare patients for discharge.
• Assist in forming and implementing patient discharge plans based on clinical assessments.
• Serve as a liaison with other departments and agencies to ensure continuity of care.
• Recommend evidence-based practices aligned with patient clinical needs.
• Arrange home health, durable medical equipment, infusion therapy, and complete referrals to appropriate agencies.
• Provide training and education about case management services.
• Collaborate with Social Work Case Managers, referring high-risk patients as needed.
• Participate in departmental coverage.

Requirements

• Bachelor of Science in Nursing (BSN).
• Registered Nurse (RN) licensure.
• Minimum of 3 years of related experience.

Skills & Knowledge

• Clinical work experience with RN licensure.
• Strong understanding of Swanson’s nursing model.
• Direct patient care experience.
• Exceptional written and verbal communication skills.
• Ability to work independently and collaboratively in a team environment.
• Proficiency in Microsoft Office and other computer systems.
• Above-average customer service skills.
• Active listening skills.
• Ability to advocate for patients and the healthcare organization to ensure continuity of care and quality clinical outcomes.

Physical Requirements

• Intermittently sitting at a computer workstation for extended periods with repetitive motions (e.g., typing).
• Occasionally standing or walking with objects weighing up to 25 pounds.
• Lifting, pushing, and/or pulling objects weighing up to 50 pounds.
• Exposure to loud noises, temperature extremes, hazardous chemicals and fumes (including waste).
• Exposure to cuts/punctures, blood-borne/airborne pathogens, and combative/violent individuals.

Schedule

• Saturday and Sunday: 7:00 a.m. to 7:00 p.m.
• Two additional 8-hour shifts during the week.

Pay

$75,364–$95,056 per year.
