Department: Continuum of Care | Location: Lexington, KY | Full-Time Staff Position

About the Role

The RN Case Manager facilitates care coordination for patients and families when clinical issues impact effective use of inpatient or outpatient treatment. Responsibilities include organizing access to tests and procedures, assisting with discharge planning, liaising with departments and agencies for care transitions, and making evidence-based recommendations for patient needs such as home health, durable medical equipment, or infusion therapy. The role also involves completing referrals, providing case management education, collaborating with Social Work Case Managers for high-risk patients, and participating in departmental coverage.

Responsibilities

  • Coordinate care for patients and families, addressing clinical challenges that affect treatment utilization.
  • Organize and facilitate access to tests, procedures, and results to prepare patients for discharge.
  • Assist in forming and implementing patient discharge plans based on clinical assessments.
  • Serve as a liaison with other departments and agencies to ensure continuity of care transitions.
  • Recommend and arrange services such as home health, durable medical equipment, or infusion therapy.
  • Complete referrals to appropriate agencies based on patient clinical needs.
  • Provide training and education about case management services.
  • Collaborate with Social Work Case Managers, referring high-risk patients as needed.
  • Participate in departmental coverage.

Requirements

  • Bachelor of Science in Nursing (BSN).
  • Registered Nurse (RN) licensure.
  • Minimum of 3 years of related experience.

Skills & Knowledge

  • Clinical work experience with RN licensure.
  • Strong understanding of Swanson’s nursing model.
  • Direct patient care experience.
  • Exceptional written and verbal communication skills.
  • Ability to work independently and collaboratively in a team environment.
  • Proficiency in Microsoft Office and other computer systems.
  • Above-average customer service and active listening skills.
  • Ability to advocate for patients and the healthcare organization to ensure continuity of care and quality clinical outcomes.

Physical Requirements

  • Intermittent sitting at a computer workstation for extended periods with repetitive motions (e.g., typing).
  • Occasional standing, walking, lifting, pushing, or pulling objects up to 50 pounds.
  • Exposure to loud noises, temperature extremes, hazardous chemicals/fumes, waste, cuts/punctures, blood-borne/airborne pathogens, and potentially combative or violent individuals.

Schedule

  • One 8-hour shift during the week.
  • Two 12-hour shifts on Saturday and Sunday (0700–1930).

Pay

$54,080–$95,056 per year.
