About the role
The HIV Nurse Case Manager plays a critical role on a dynamic, multidisciplinary HIV primary care team that has been nationally recognized for excellence. The role involves providing high-quality nursing care, conducting clinical assessments, and managing chronic diseases alongside other team members.
Responsibilities
- Conducts outpatient primary care nursing including medical assessments and triage services for HIV team patients.
- Administers vaccines, injectable medications, and provides wound assessment and care.
- Coordinates care with integrated HIV team behavioral health staff.
- Visits patients at shelters and other outreach sites to provide nursing care and care coordination services.
- Supports chronic disease management and medication adherence for HIV and other co-morbid conditions.
- Identifies and addresses structural barriers to care and medication adherence.
- Collaborates with medical providers to coordinate follow-up related to medication management.
- Provides education and preventative services to HIV team patients.
- Ensures a compassionate and therapeutic environment for patients and the care team.
- Trains new nursing staff as directed by supervisors.
Qualifications
- Licensed as a Registered Nurse in the Commonwealth of Massachusetts.
- Bachelor’s degree in nursing preferred; 2 years of experience working in an ambulatory/outpatient care environment required.
- HIV primary care experience strongly preferred.
- Certification as AIDS Certified Registered Nurse (ACRN) preferred, but can be obtained after hiring.
- Expertise in substance use disorder and risk reduction principles.
- Experience working with people experiencing homelessness.
- Mental Health experience, including knowledge of trauma informed care.
- Experience working in a community health center setting.
- Phlebotomy experience preferred or training upon hire.
Compensation and Benefits
- Compensation increases based on years of experience, ranging from $40.00 - $58.00 hourly.
