Nurse Case Manager I

Make a difference for Vermonters with complex health and social needs, working with community-based teams and systems, as a RN Nurse for the Vermont Chronic Care Initiative (VCCI) with the Agency of Human Services (AHS).

About the role

This position primarily covers Windsor County, but flexibility to cover other counties may be requested. The role involves patient-centered clinical case management to support health and health-related outcomes for individuals with complex health and social needs. Community-based case management and field-based visits occur in various settings, including private apartments, motels, correctional facilities, encampments, and shelters.

You will report to the AHS Field Director and Central Office Administration, collaborating closely with an array of partners and providers in the health and social services arena. The position uses a team-based care approach, incorporating patient engagement tools, care teams, shared care plans, a strength-based approach, and motivational interviewing skills.

Responsibilities

• Provide patient-centered clinical case management to support health and health-related outcomes for individuals with complex needs.
• Conduct community-based case management and field-based visits in settings such as private apartments, motels, correctional facilities, encampments, and shelters.
• Work closely with diverse stakeholders, including individuals, family members, healthcare disciplines, and service providers.
• Integrate operationally with various healthcare reform initiatives.
• Utilize effective communication, clinical experience, and knowledge of community culture to build working relationships.
• Promote personal and client safety through critical assessment skills.
• Perform duties outside of normal work hours as needed.

Environmental Factors

• Duties are performed in facilities, standard office settings, and/or in the community.
• Extensive travel may be required; a reliable private means of transport is necessary.
• Some resistance or objection to standards may be anticipated, requiring tact and diplomacy.

Requirements

• Possession of (or eligibility for) licensure as a Registered Nurse (RN) in Vermont or eligibility to practice in Vermont via a multi-state license.
• Three (3) or more years of professional nursing experience in an acute hospital setting, long-term care, health insurance carrier, or within a community health/public health setting.
• Must maintain Vermont or multi-state licensure as a Registered Nurse as a condition of employment.
• Reliable personal transportation for work outside the office or in community and field-based settings.

Preferred Qualifications

• Bachelor of Science in Nursing (BSN).

Schedule

• Monday–Friday schedule.
• Eleven paid state holidays each year.
