Responsibilities

• Comprehensive assessment, planning, coordination, implementation, and evaluation of care
• Collaboration with injured workers, insurance carriers, medical providers, employers, and attorneys
• Monitoring clinical progress and outcomes and reporting to stakeholders
• Managing complex and catastrophic cases aligned with your expertise

Qualifications

• Current Registered Nurse (RN) license in Georgia
• Nationally recognized case management certification: CCM, CRRN, COHN, or CDMS
• GA Rehabilitation Supplier registration
• Valid driver’s license with good driving history

• Comfort working independently with minimal supervision
• Proficiency with technology and documentation systems
