Responsibilities

• Conduct medical case management, including screening, education, and counseling
• Provide medication management
• Coordinate with community healthcare providers
• Participate in a rotating on-call schedule, primarily offering remote phone consultations to ensure accessibility

Requirements

• Bachelor's or Associate Degree in Nursing
• Current Illinois RN licensure
• Valid driver's license
• Experience with psychiatric care and youth preferred
