Nurse (Case Management)

About the role

This position provides advanced professional nursing case management and care coordination services to eligible American Indian and Alaska Native beneficiaries.

Responsibilities

• Helps manage an assigned caseload of patients with chronic or recurring health conditions that require case management services.
• Develops and implements individualized, patient-centered care plans in collaboration with patients, families, and the healthcare team.
• Participates in discharge planning activities for assigned patients, including identifying discharge needs, coordinating post-discharge services, and providing patient and family education.
• Collaborates to coordinate care across providers, disciplines, and care settings to ensure continuity and prevent fragmentation of services.
• Provides direct professional nursing care to assigned patients with chronic and recurring health conditions.

Requirements

• U.S. Citizenship is required.
• Selectee may be subject to a probationary period.
• Must successfully pass the E-Verify employment verification check.
• May be subject to a background investigation.
• Must complete a federal background investigation as a condition of employment.
• Must successfully complete a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.
• Must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

Qualifications

• A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required.
• Successfully complete a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.
• Must have passed the National Council Licensure Examination.
• Must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

Skills

The selected individual is required to obtain and maintain medical staff clinical privileges, including any licensure requirements.

Benefits

A housing subsidy of up to $900 per pay period may be available.
