About the Role
The Nurse Care Manager supports residents in maintaining independence and safety through holistic, individualized care. This on-site position at a Chelsea housing community requires fluency in Spanish to serve a predominantly Spanish-speaking population. The role involves active outreach, building trusting relationships, tracking residents with special needs, and educating staff on health issues.
Responsibilities

Requirements
- RN with 1+ year of experience or LPN with 3+ years of experience
- Fluency in Spanish
- Travel within the Boston area required
