Nurse Care Manager

Qualifications

• Basic Requirements:
• English Language Proficiency
• Graduate of a school of professional nursing approved by an accrediting agency and accredited by ACEN or CCNE
• Individuals in a master's level bridge program may become registered as a nurse with a state licensing board before completing the program
• Current full, active, and unrestricted registration as a graduate professional nurse in the United States or the District of Columbia
• Credit for foreign nursing education higher than an associate degree requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA
• Grandfathering Provision: All persons currently employed in VHA in 0610 series and performing duties as described in the qualification standard on 1/29/2024 are considered to have met all qualification requirements for the grade held including positive education and licensure/certification
• Exception: Candidates who otherwise meet the basic education requirements but do not possess the required licensure may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis)

Duties

• Leads and organizes delivery of care to ensure continuity of care and peer accountability for practice in alignment with the ANA Standards of Nursing Practice
• Applies advanced clinical knowledge/judgment to lead staff involvement in planning, decision-making, and evaluating outcomes beyond the immediate practice setting
• Applies nursing expertise to lead improvement, with demonstrated outcomes, beyond the immediate practice setting
• Demonstrates ability to be self-directed in using the nursing process to improve the outcomes of care at the program/service level
• Applies collaborative team approach in identifying, analyzing, resolving problems, and sustaining improvements
• Applies problem-solving skills beyond the unit level, leading to improved care/outcomes
• Leads and directs others in organizing the delivery of care to assure continuity of care and peer accountability for practice
• Evaluates program/process or medical center policies/procedures and reports effectiveness or provides leadership, analyzing and addressing ethical legal issues that impact patients and staff and the medical center
• Demonstrates knowledge and compliance of Ethical Resolution Process
• Assumes responsibility and demonstrates accountability for all professional nursing actions
• Provides considerate care, respecting patients'/staff's personal values and belief system in provision of care
• Safeguards privacy and maintains confidentiality of all patient information including electronic and print
• Serves as a resource and advocate for the protection of patient/family rights
• Initiates and participates in consultations by analyzing ethical issues, appropriate theoretical principles, and provides guidance as appropriate
• Models ethical conduct with other staff
• Analyzes available resources, identifies risk areas which include budgetary, staffing, supplies and equipment, or availability of data/information
• Evaluates options available and directs a course of action which will result in the most favorable patient, program, or organizational outcomes
• Establishes patient care priorities that ensure safe, efficient, and effective patient care
• Provides care based on patient needs that can be delivered in a safe, efficient, and cost-effective manner
• Evaluates and assigns care based on staff competencies and priorities ensuring effective patient care with optimum use of all assigned resources
• Identifies all aspects of patient care delivery and utilizes appropriate resources to enhance patient outcomes
