Purpose Statement/Position Summary
The Nurse Care Manager performs complex discharge planning for patients of different ages and developmental levels with serious illnesses. Focuses on patients with complexities related to finance and/or resource utilization. Acts as advocate for the patient/family by collaborating with the multidisciplinary team to develop and implement a case management plan to ensure quality patient outcomes, and customer satisfaction in a cost effective manner. Participates/facilitates in the development and outcome analysis of clinical pathways as a member of the multidisciplinary CQI team.
Minimum Qualifications/Work Experience
- 3+ years nursing experience with advanced level pediatric clinical experience and utilization management and/or discharge planning experience.
- Thorough knowledge of managed care, utilization management, discharges planning, and continuous quality improvement concepts.
- Computer Literacy.
- Strong interpersonal and negotiation skills.
- Fluency in both English and Spanish desired.
Education/Licensure/Certifications
- Bachelor’s Degree in Nursing required.
- Master’s degree in nursing, Health Care Administration, or related field preferred.
- CURRENT Registered Nurse licensure in the State of California.
- CURRENT BLS form the American Heart Association.
