Nurse Care Coordinator - RN

The San Jose Mission Clinic has provided free healthcare services to uninsured and underserved community members for more than two decades. Located on the San Jose Mission campus, the clinic offers primary care, chronic disease management, and specialty services through the dedication of its staff and volunteer physicians. As a nurse in this role, you'll have the opportunity to make a meaningful impact by delivering compassionate care to those who need it most while serving a mission-driven community.

Responsibilities

• Responsible for medical intake procedures, maintaining daily patient flow, assisting with organization of patient care services, and laboratory and phlebotomy services.
• Maintain clinic compliance with governmental agency standards.
• As part of a multidisciplinary team, coordinates the provision of all medical services.
• Assures appropriateness of patient care, facilitates case management, encourages appropriate utilization of resources, and ensures timely specialty consults.
• Provides education to other team members.
• Perform other duties as assigned.

Requirements

• Associate’s degree in Nursing or Nursing Diploma.
• Active RN (Registered Nurse) license.
• 3 years of clinical-related experience.
• Proficient in English and Spanish (highly preferred).
• This position requires Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA).
