About the Role
The Advanced Practice Clinician (NP/PA) is service/physician aligned, managing a distinct patient population. The role may require performing outpatient and inpatient care, facilitating discharges, and ensuring follow-up. The APC ensures adherence to clinical pathways and protocols while maintaining an environment of safe, quality care. This position demonstrates advanced medical and nursing knowledge, clinical competence, sound judgment, and professional communication skills.
Responsibilities
- Perform and document complete history and physical examinations for acute and complex chronically ill patients in the medical record.
- Order and collect data using appropriate assessment techniques, including diagnostic information and procedures.
- Serve as the first responder for patient status changes, orders, and situations requiring procedural and resuscitative interventions.
- Assess risks associated with patient care, including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, and invasive interventions.
- Coordinate daily clinic schedules with the attending physician team, present patient reports, and collaborate on the plan of care for assigned patient populations.
- Formulate and document individualized plans of care based on diagnoses, utilizing evidence-based practice guidelines or protocols.
- Prescribe diagnostic strategies and therapeutic interventions, including pharmacologic, non-pharmacologic, and surgical approaches.
- Manage diagnostic tests through ordering, interpretation, performance, and supervision.
- Perform advanced procedures consistent with privileges and competency validation, such as minor suturing, complex wound management, and line/tube placements and removals.
- Maintain ongoing communication and collaboration with the interdisciplinary healthcare team.
- Facilitate communication with patients, families, and staff to promote continuity of care.
- Document key components of patient progress, including daily progress notes, transfer/discharge summaries, and procedure notes, ensuring compliance with critical care standards.
- Demonstrate professional practice behaviors, including precepting/mentoring students, nursing staff, and novice APCs.
- Seek opportunities for active engagement in research and evidence-based practice analysis.
- Actively participate in Grand Rounds, APN council meetings, faculty/unit meetings, and M&M presentations.
- Maintain CEUs and membership in a professional organization.
Requirements
- Licensed Nurse Practitioner (LIC-NP) or Physician’s Assistant Certified (LIC-PA) in Tennessee.
- Minimum of 1 year of relevant work experience.
- Master’s degree in a relevant field.
