NP/PA Per Diem - Port St. Lucie area, FL

About the Role

Optum Home & Community Care, part of the Optum family of businesses, is creating a new integrated care model that addresses physical, mental, and social needs, helping patients access and navigate care anytime and anywhere. As a team member of our Senior Community Care (SCC) team, you'll provide care to patients in nursing homes, senior housing, and assisted living settings, improving access to care and creating a seamless health journey for patients across settings.

Responsibilities

• Conduct comprehensive assessments, including past medical history review, medication reconciliation, vital signs, physical exam, evidence-based screenings, and point-of-care testing.
• Support members during short-term sub-acute rehabilitation episodes.
• Ensure accurate and complete documentation, including ICD-10 conditions.
• Communicate and collaborate with the interdisciplinary care team.
• Conduct advanced illness and advanced care planning conversations.
• Provide patients and caregivers with counseling and education.

Requirements

• 2+ years of NP or PA clinical experience (long-term care setting preferred).
• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care (for NPs) or current NCCPA certification and state licensure (for PAs).
• Active, unrestricted licensure in the state of practice without disciplinary actions or practice limitations.
• Active and unrestricted driver's license with access to reliable transportation.
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire.
• Ability to gain a collaborative practice agreement, if applicable in your state.

Qualifications

• Experience in geriatric medicine, long-term care, senior living, or home care setting.
• Understanding of geriatrics, chronic illness, advanced illness, and end-of-life discussions.
• Proficiency with electronic medical records and technology.

Pay

Compensation for this role generally ranges from $53.00 - $79.00 per hour. Total cash compensation includes base pay and bonus, based on factors such as local labor markets, education, and work experience. We comply with all minimum wage laws as applicable.

$3,500 Sign-on Bonus for external candidates and eligible internal candidates. Per Diem: 1 shift per week (4 per month) minimum commitment. No on-call, weekends, or holidays required.
