About the Role

We are seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health assessments for Medicare Advantage and other members. The role focuses on improving health outcomes through education, gap closure, and collaboration with primary care providers, without prescribing authority.

Responsibilities

  • Perform comprehensive medical history reviews, physical exams, health risk screenings, and point-of-care tests
  • Identify and document diagnoses for care management
  • Coordinate with primary care providers (PCPs) to address care gaps
  • Educate members on health topics, medications, and preventive care
  • Address social determinants of health and provide appropriate referrals
  • Recognize urgent or emergent situations and intervene appropriately

Requirements

  • Active, unrestricted licensure in the practicing state
  • Valid driver’s license and reliable transportation
  • Ability to navigate varied home environments and perform assessments
  • For Nurse Practitioners: Certification from ANCC or AANP
  • For Physician Assistants: NCCPA certification and state licensure

Preferred Qualifications

  • 1+ years of clinical experience
  • Proficiency with electronic medical records (EMRs)
  • Ability to transport and use assessment equipment
  • Strong communication skills with diverse populations

Schedule
