About the role
Join an innovative healthcare team as a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant).
Responsibilities
- Conduct comprehensive in-home health assessments for Medicare Advantage members, including medical history review, physical exams, screenings, and point-of-care testing.
- Identify diagnoses, document findings, and communicate with primary care providers to address care gaps.
- Recognize urgent situations, provide education on disease management, medications, and social determinants of health, and make appropriate referrals.
Requirements
- Active licensure in the state of practice, with certification as an NP or PA.
- Reliable transportation and ability to navigate varied home environments.
Qualifications
- Preferred: clinical experience in family, geriatric, or home health care.
- Preferred: proficiency with EMRs and strong communication skills.
