NP or PA - Statewide Traveler NY- Care at Home

About the Role

Provides in-home clinical care to high-risk patients as part of an interdisciplinary team, focusing on preventive, acute, and chronic condition management. Travel throughout the state of New York is required, with no night, weekend, or holiday shifts.

Responsibilities

• Conduct comprehensive in-home assessments, including medical history, medication reconciliation, vital signs, and physical exams.
• Manage acute medical situations and exacerbations of chronic conditions with diagnostics and interventions.
• Support short-term rehabilitation episodes, document clinical findings accurately, and communicate with team members.
• Engage in advanced care planning and provide education and counseling to patients and caregivers.

Requirements

• Active, unrestricted licensure in the practicing state; DEA license or ability to obtain.
• For Nurse Practitioners: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• For Physician Assistants: Current NCCPA certification and state licensure.
• Reliable transportation and ability to travel up to 85% of the time.

Preferred Qualifications

• 1+ years of clinical experience, especially in geriatrics, home health, or long-term care.
• Proficiency in electronic medical records.
