NP or PA - Statewide Traveler NY- Care at Home

About the Role

Provide in-home clinical care as part of an interdisciplinary team, focusing on high-risk patients in their residences. Conduct comprehensive assessments, manage acute and chronic conditions, and facilitate short-term rehabilitations. Collaborate with care teams, document accurately, and engage in advanced care planning and patient education.

Travel extensively within the state, up to 85%, with home on weekends and no night or holiday shifts.

Requirements

• Active, unrestricted licensure and certification (ANCC/AANP for NPs, NCCPA for PAs).
• Valid driver’s license, reliable transportation, and DEA prescriptive authority or ability to obtain.
• Ability to navigate varied environments and perform physical duties.

Preferred Experience & Skills

• 1+ years in clinical settings such as geriatrics, home health, or long-term care.
• Experience with complex behavioral, social, or functional needs.
• Proficiency with EMRs.
• Knowledge of geriatrics, chronic illness, and end-of-life discussions.

Pay

Salary ranges from $125,000 to $175,000, including base pay and bonuses.
