About the role
This is a flexible, part-time assessment role requiring approximately 10 hours weekly during daytime hours, focusing on providing comprehensive health assessments to patients in transitional care settings.
Responsibilities
- Collaborate with primary care physicians, nursing staff, families, and case management teams to deliver quality care
- Conduct thorough health assessments, document findings, and support care coordination
- Ensure accurate coding, documentation, and closure of gaps in care
Requirements
- Active, unrestricted license in the state of practice
- Certified Nurse Practitioner or Physician Assistant with relevant certifications
- Ability to obtain prescriptive authority and collaborate within state regulations
- Reliable transportation, physical ability to navigate various dwelling conditions, and availability for 10 hours weekly
Skills
- Preferred experience in Long Term Care, geriatrics, and chronic illness management
- Proficient in electronic medical records and developing positive patient relationships
Pay
Pay ranges from $90.00 to $155.00 per visit, with earnings based on the number of visits.
