Responsibilities

• Evaluate patients and develop individualized treatment plans
• Improve strength, balance, mobility, and functional independence
• Educate patients and caregivers on safety and fall prevention
• Collaborate with physicians and the interdisciplinary team to maximize patient outcomes
• Help patients safely recover and remain independent in their own homes
• Build meaningful relationships with patients and families while promoting independence, improving outcomes, and ensuring compassionate, patient-centered care

Requirements

• Current California Physical Therapist license
• Current BLS certification
• Valid driver’s license, reliable transportation, and auto insurance
• Strong clinical assessment and treatment planning skills
• Ability to work independently while collaborating with an interdisciplinary team
• OASIS documentation experience

Qualifications

• Previous experience working in home health, hospice, or a similar caregiving environment
• Familiarity with KanTime Electronic Medical Record (EMR) system
• Bilingual skills are a plus

Pay

Salary Range: $58.00–$63.00 per hour (based on experience)
