Salary Range: $58.00-$63.00 per hour – based on experience
About the role

As a Physical Therapist (PT), you will evaluate patients, develop individualized treatment plans, improve strength, balance, mobility, and functional independence, and educate patients and caregivers on safety and fall prevention. You will collaborate with physicians and the interdisciplinary team to maximize patient outcomes, helping patients safely recover and remain independent in their own homes. In this role, you will build meaningful relationships with patients and families while promoting independence, improving outcomes, and ensuring compassionate, patient-centered care.
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Improve patient strength, balance, mobility, and functional independence
- Educate patients and caregivers on safety and fall prevention
- Collaborate with physicians and the interdisciplinary team to maximize patient outcomes
- Provide compassionate, patient-centered care in patients’ homes
Requirements
- Current California Physical Therapist license
- Current BLS certification
- Valid driver’s license, reliable transportation, and auto insurance
- Strong clinical assessment and treatment planning skills
- Ability to work independently while collaborating with an interdisciplinary team
- OASIS documentation experience
Qualifications
- Previous experience working in home health, hospice, or a similar caregiving environment (preferred)
- Familiarity with KanTime Electronic Medical Record (EMR) system (preferred)
- Bilingual skills (a plus)
