Note Reviewer

About the role

Liaisons Community Care is seeking a detail-oriented Note Reviewer to review behavioral health documentation for accuracy, completeness, and compliance. This remote role works with clinical and administrative staff to identify documentation gaps and help improve the quality of client records.

Responsibilities

• Review progress notes, treatment plans, assessments, and discharge summaries for required information, clarity, and consistency.
• Check that documentation reflects the services provided, treatment interventions, and progress toward goals.
• Verify required signatures, dates, client identifiers, and billing information.
• Identify missing information or discrepancies and provide clear, constructive feedback to staff.
• Track review findings and follow up on needed corrections.
• Support documentation audits and quality improvement efforts.
• Protect confidential client information and follow HIPAA requirements.

Qualifications

• Bachelor’s degree in Healthcare Administration, Psychology, Social Work, or a related field required; master’s degree preferred.
• Experience reviewing healthcare documentation required; behavioral health experience preferred.
• Knowledge of behavioral health documentation standards, DSM5 terminology, CPT codes, and billing documentation requirements.
• Strong attention to detail, sound judgment, and clear written communication.
• Ability to manage priorities and work independently in a remote setting.
• Experience using electronic health record systems.
• Ability to meet applicable background screening requirements.
