Overview
Serving more than 25 counties, BJC Home Care has become the largest home care network in the region and one of the largest in the country. The Home Infusion Therapy Department of BJC Home Care Services works closely with physicians, patients and caregivers to enable patients to remain or return home while receiving intravenous medications.
Role Purpose
The Intake Coordinator (non-clinical) is responsible for coordinating patient services during the home care intake process. This position manages all requests received via multiple work queues, monitors potential patient admission dates and determines their eligibility for services. This position works with internal and external customers (patients, physician/referral sources, case managers, etc.) to ensure an outstanding customer experience.
Responsibilities
- Performs complete and accurate data entry of patient information; processes referrals and ensures that patient information, records and other documents are updated appropriately; assists external and internal customers on entering home health referrals correctly.
- Identifies and resolves problems encountered during the home health referral/order entry and confirmation processes; handles customer complaints received via phone and email; escalates problems and concerns as necessary.
- Maintains and updates patient information; monitors all work queues associated with the Central Referral Center; understands and processes all referral types; triages phone calls to other departments to ensure a good customer experience; passes along referrals/information to other services as needed to ensure that the comprehensive needs of the patient are met.
- Learns and becomes proficient in the use of all clinical platforms used by BJC (EPIC/ECIN/ON Base); participates in performance improvement initiatives & activities; helps to streamline and standardize work flows & processes; assists with the training and support of new staff members.
- Works closely with Authorization and Benefit departments, ensuring patient benefits and coverage parameters are accurate and communicated to the appropriate staff.
Minimum Requirements
- Education: High School Diploma or GED
- Experience: No Experience Preferred
- Education: Associate's Degree
- Experience: 2-5 years
