About the role

The NJ - Nurse Manager provides leadership, oversight, and support to a team of traveling Registered Nurses delivering care in patient homes and community-based settings. This role ensures clinical excellence, operational efficiency, and compassionate patient-centered care aligned with Ponos Care’s mission and clinical standards. The Nurse Manager collaborates closely with clinical leadership to ensure nursing staff are supported, care delivery is coordinated effectively, and patient outcomes reflect the highest standards of quality and safety.

Responsibilities

  • Manage day-to-day operations of traveling RN team supporting home-based patient care
  • Coordinate nursing schedules, assignments, and patient visits to ensure optimal care delivery
  • Monitor clinical workflows and ensure timely documentation in EMR systems
  • Support implementation of care delivery protocols and treatment plans
  • Provide leadership, coaching, and mentorship to nursing staff
  • Conduct performance evaluations and provide ongoing feedback
  • Promote professional development and clinical competency improvement
  • Foster a collaborative and supportive work environment
  • Ensure delivery of high-quality, culturally competent, and patient-centered care
  • Monitor patient care plans and assist with complex case management as needed
  • Collaborate with interdisciplinary teams to improve patient outcomes
  • Support continuous improvement of care coordination practices
  • Coordinate patient assignments to ensure efficient utilization of clinical resources
  • Oversee home visit schedules across multiple geographic areas
  • Ensure appropriate staffing coverage for patient care needs
  • Address scheduling challenges and adjust resources as needed
  • Support development and implementation of patient care programs, policies, and procedures
  • Ensure compliance with clinical regulations and healthcare standards
  • Monitor adherence to documentation standards and quality measures
  • Escalate clinical or operational risks to leadership as appropriate
  • Partner with clinical leadership to identify challenges and opportunities for improvement
  • Communicate operational and clinical updates to Director of Clinical Services and CNO
  • Collaborate with interdisciplinary care teams to ensure coordinated care delivery
  • Promote a culture of respect, inclusion, and professionalism
  • Ensure nursing program supports home-based care model
  • Align nursing services with Ponos Care’s value-based care approach
  • Develop care delivery strategies that improve patient outcomes
  • Support initiatives that reduce hospital readmissions
  • Ensure nurses follow clinical documentation standards
  • Maintain HIPAA compliance
  • Ensure nursing staff follow scope of practice rules
  • Ensure proper certifications and licensure are current
  • Ensure nursing schedules are efficient
  • Reduce missed patient visits
  • Improve care coordination processes
  • Monitor nurse productivity and workload balance
  • Provide coaching to RNs
  • Conduct performance reviews
  • Support training and clinical development
  • Improve nurse retention and engagement
  • Implement telehealth workflows
  • Support new clinical programs
  • Identify opportunities to improve patient care delivery
  • Contribute ideas to improve care coordination model

Requirements

  • Bachelor’s degree in Nursing or related field
  • Active Registered Nurse (RN) license in applicable state(s)
  • Current certifications such as BLS, ACLS, CPR (as applicable)
  • 3+ years clinical experience in acute care, infusion therapy, pain management, hematology, or community health
  • 1+ years of nursing leadership or supervisory experience
  • Experience coordinating care in home health or mobile healthcare environments preferred
  • Ability to travel to patient locations as needed
