About the role

Human Touch Home Health is seeking a licensed New Graduate Physical Therapist to join our growing team serving patients throughout Fredericksburg, Stafford, Spotsylvania, King George, Caroline County, and surrounding communities. As a Home Health Physical Therapist, you'll work one-on-one with patients in their homes, developing personalized treatment plans that help them regain strength, improve mobility, and return to the life they love.

Responsibilities

• Complete physical therapy evaluations and reassessments
• Develop individualized treatment plans based on patient goals and clinical needs
• Provide skilled physical therapy services in patients' homes
• Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, and home safety
• Monitor patient progress and adjust treatment plans as needed
• Collaborate with physicians and the interdisciplinary care team
• Complete timely and accurate clinical documentation using WellSky

Requirements

• Active Virginia Physical Therapist License
• New Graduates Are Encouraged to Apply
• Valid Driver's License and Reliable Transportation
• Strong Communication and Interpersonal Skills
• Passion for Delivering Compassionate, Patient-Centered Care
• Home Health Experience Not Required—We'll Provide the Support You Need to Succeed
