About the Role

The primary purpose of this position is to provide assigned residents with assistance in meeting scheduled needs as outlined in the care plan/service plan and to assist with unscheduled needs when required, according to established facility policies and procedures. As a Caregiver, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties. While every effort has been made to outline your duties comprehensively, additional related tasks may be assigned as needed.

This role requires a North Dakota Certified Nurse Aide (CNA) license. For Certified Medication Aide (CMA) responsibilities, a North Dakota CMA certificate/license and knowledge in medication administration are required.

Responsibilities

  • Documentation
    • Consistently record entries on all required forms, including ADL forms, Medication Administration Records (MAR), and care/service plans.
    • Ensure all documentation reflects the resident’s current condition and report exceptions or problems on the 24-hour report sheet.
  • Admission, Transfer & Discharge of Resident
    • Greet residents and families in a friendly and welcoming manner.
    • Orient residents and families to community routines, including fire evacuation procedures and meal times.
    • Obtain admit height, weight, and vital signs.
    • Assist residents with unpacking or packing belongings.
  • Medication Assistance
    • Obtain medication certification and follow all applicable state rules and laws for medication assistance.
    • Assist residents with medication administration according to facility policies and established standards of practice.
  • Personal Care
    • Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, mouth care, and toileting as outlined in the service/care plan.
    • Perform delegated nursing care, including taking and recording blood pressure, pulse, respirations, height, and weight.
    • Report any changes in resident’s condition, including skin breakdown or behavioral changes.
  • Instrumental Activities of Daily Living
    • Assist residents with meal preparation, housekeeping, cleaning of residence, and laundry.
    • Support residents in the use of telephone and other communication needs.
  • Montessori Dementia Programming
    • Become certified and maintain certification in Montessori Programming.
    • Implement Montessori programming concepts in activities and individualized service/care plans.
    • Perform Montessori-Based Dementia Programming with residents across all service programs.
    • Assist in developing and implementing Montessori-Based activity programs and activities (e.g., baking, crafts).
    • Lead resident activity groups (e.g., exercise, manicures).
  • Appearance
    • Wear appropriate clothing as described in the Dress Code policy.
    • Maintain clean clothing/uniform and shoes.
    • Wear a name tag while on duty.
  • Communication
    • Identify and requisition supplies as needed.
    • Seek clarification of assignments and ask questions when needed.
    • Volunteer assistance to co-workers as time permits or as directed.
    • Treat co-workers, residents, families, and guests with courtesy, respect, and in a supportive manner.
    • Resolve interpersonal conflicts directly and privately, seeking supervisor assistance as necessary.
    • Maintain resident-centered conversations and a customer-friendly environment that values and respects each individual.
  • Education/Training
    • Attend department meetings, all-staff meetings, mandatory in-services, and training.
    • Understand and use Universal Precautions, OSHA Bloodborne Pathogen Standard, and follow established infection control, hazardous communication, and safety rules.
  • Infection Control
    • Demonstrate knowledge of infection control practices by complying with established policies and procedures.
    • Wash hands when gloves are removed and when soiled; use hand sanitizer when appropriate.
    • Use universal precautions, including Personal Protective Equipment (PPE) and Blood and Body Fluid spill kits when indicated.
    • Dispose of waste products properly.
  • Maintains Safe/Sanitary Environment
    • Use proper body mechanics when performing tasks.
    • Report hazardous conditions or equipment to the manager immediately.
    • Handle equipment safely and properly.
    • Follow emergency safety policies and procedures (e.g., fire, evacuation, elopement).
    • Report accidents and incidents immediately to the manager and complete incident reports.
    • Follow established safety precautions and safe operating procedures.
  • Resident Rights
    • Address residents by their preferred name and avoid labels such as "sweetie" or "dear."
    • Discuss resident status in private and maintain confidentiality.
    • Treat residents courteously, politely, and in a physically appropriate manner.
    • Respond to family/resident complaints consistent with facility policy.
    • Respect and uphold resident rights at all times.
  • Resident Safety/Concerns
    • Document resident behaviors on the Resident Care & Concern Form and report to the manager.
    • Report emergent resident concerns to the manager immediately.
    • Document non-emergent concerns on the Health & Wellness Concern Form.

Requirements

  • Minimum of an 8th-grade education.
  • Willingness to become a Certified Medication Aide (CMA) in accordance with state laws.
  • Certified as a Nurse Assistant or listed as a Unlicensed Assistive Person (UAP) on the NDBON registry.
  • Minimum age of sixteen (16) years.
  • Ability to read, write, speak, and understand English, and follow oral and written instructions.
  • Ability to make independent decisions when circumstances warrant such action.
  • Ability to deal tactfully with personnel, residents, family members, visitors, and government agencies.
  • Patience, tact, cheerful disposition, enthusiasm, and willingness to handle difficult residents.
  • Ability to accurately report information concerning a resident’s condition.

Physical and Sensory Requirements

  • Ability to move intermittently throughout the workday.
  • Ability to speak and write English clearly and understandably.
  • Ability to cope with mental and emotional stress associated with the position.
  • Ability to see and hear (or use prosthetics to enable these senses) adequately to meet position requirements.
  • Ability to function independently, demonstrate flexibility, personal integrity, and work effectively with residents, personnel, and support agencies.
  • Good general health and emotional stability.
  • Ability to relate to and work with ill, disabled, elderly, emotionally upset, and potentially hostile individuals.
  • Ability to stoop, turn, bend, stretch, crawl, kneel, reach overhead, and lift, push, pull, or move a minimum of fifty (50) pounds.
  • Frequent standing, walking, and occasional sitting.
  • Ability to smell, discriminate speech, speak clearly, discriminate color, and possess intact peripheral vision.
  • Ability to assist in the evacuation of residents.
  • Occasional work with toxic chemicals, sprays, or mists.
  • Frequent use of hands, arms, legs, and feet, including the use of gloves.
  • Ability to balance and demonstrate rapid mental-muscular coordination.
  • Occasional use of a face shield or facial protection.

Working Conditions

  • Works throughout resident areas, including med rooms, offices, resident condominiums, common spaces, and health areas.
  • Frequent sitting, standing, bending, lifting, walking, and moving during working hours.
  • Subject to frequent interruptions and interactions with residents, personnel, visitors, and government agencies under various conditions.
  • Exposure to falls, burns from equipment, odors, and other environmental factors throughout the workday.
  • Potential exposure to hostile or emotionally upset residents, family members, personnel, and visitors.
  • May be required to work beyond normal hours, including weekends, holidays, and temporarily in other positions as needed.
  • Exposure to infectious waste, diseases, and conditions, including HIV, Hepatitis B (HBV), and Hepatitis C (HCV) viruses.
  • Communication with other department personnel.
