The Opportunity
The Navigator I plays a critical role within Eon’s Care Management program. This position engages patients and providers at pivotal moments when timely follow-up matters most. Using the EPM platform, Nav 1s support patient follow-up by communicating with patients and provider offices, coordinating appointment logistics, and tracking adherence to care pathways.
Qualifications and Skills
- Associate degree or higher in a healthcare-related field is preferred
- Healthcare experience required (minimum 2 years in a medical setting preferred)
- Foundational knowledge of medical terminology (required)
- Proficiency with Apple MacBook and Google Suite (preferred)
- Excellent verbal and written communication skills
- Able to work in a fast-paced, high-volume environment
- Highly organized with the ability to manage multiple tasks and meet deadlines
