Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona, including residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.
About the role
The Navigator I enhances the likelihood of success for clients by identifying and reducing barriers, connecting them with needed services and resources. This role works with individuals requiring additional support and engagement, conducting outreach, case management, and ongoing wrap-around supportive services. The Navigator I maps out appropriate next steps for clients needing assistance accessing various services and is part of a multi-disciplinary team. Responsibilities include documenting and tracking patient interactions according to regulatory and organizational standards.
Responsibilities
- Conduct outreach and engagement with clients in need of additional support.
- Provide case management and ongoing wrap-around supportive services.
- Identify and reduce barriers to client success.
- Connect clients with necessary services and resources.
- Map out appropriate next steps for clients accessing services.
- Document and track required patient interactions according to Arizona Administrative Code (AAC), ADHS/DBHS Provider Manual, CARF, Community Bridges Policies and Procedures, and program-specific requirements.
Requirements
- High school diploma or GED (required).
- Associates Degree (or higher) in a field related to Behavioral Health (preferred).
- Minimum of six months of recovery from substance use and/or mental health disorders (required).
- 1-3 years of full-time healthcare-related work experience (preferred).
- 1-3 years of substance abuse/mental health recovery-oriented service (preferred).
- Current Arizona Driver’s License (valid and in good standing).
- Clear 39-month Motor Vehicle Record.
- Arizona Fingerprint Clearance card or good cause exemption (required).
