MSW Social Worker – Transitional Care

About the role
Provides proactive social work and care management services for patients across the lifespan, collaborating with the multidisciplinary care team to address psychosocial needs, discharge planning, barriers to care, financial and housing concerns, placement coordination, custodial issues, and behavioral health referrals. Assists patients and families with supportive counseling, resource linkage, and access to community services, while managing swing bed admission assessments and patient disposition planning.
Responsibilities
- Provide social work and care management services for patients of all ages
- Collaborate with multidisciplinary care team on psychosocial needs and discharge planning
- Address barriers to care, financial and housing concerns, and placement coordination
- Assist with custodial issues and behavioral health referrals
- Offer supportive counseling and resource linkage to patients and families
- Manage swing bed admission assessments and patient disposition planning
Requirements
- Master of Social Work (MSW)
- 2 years’ experience in transitional care in a hospital setting
Preferred Qualifications
- Basic Life Support (BLS) certification
Schedule
13-week assignment • 5×8 or 4×10 shifts • Monday–Friday days
