Responsibilities
- Aids the patient and family with health-related financial, social, and emotional concerns.
- Provides social work services in accordance with the plan of care.
- Communicates changes in the patient's clinical status to other healthcare team members and care center office staff.
- Educates the patient and family members and other team members on medical social work interventions and treatment plans.
- Performs care planning, transfer, and discharge functions.
- Identifies risks or threats to the patient's health and implements appropriate interventions.
- Communicates identified risks/threats to the physician and other members of the healthcare team.
Qualifications
- Holds a master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education.
- Has one year of social work experience in a healthcare setting.
- Mets state and federal requirements for this position.
- Currently holds a CPR certification.
Pay
$75,000.00 - $82,000.00 salary
Schedule
