MSW Medical Social Worker Hospice

Responsibilities

• Aids patients and families with health-related financial, social, and emotional concerns.
• Provides social work services aligned with the patient’s care plan.
• Communicates changes in the patient’s clinical status to healthcare team members and care center office staff promptly.
• Educates patients and family members, as well as other team members, on medical social work interventions and treatment plans.
• Performs care planning, transfers, and discharges.
• Identifies risks or threats to the patient’s health and implements appropriate interventions.
• Communicates identified risks/threats to the physician and other healthcare team members to coordinate care effectively.

Qualifications

• Holds a master’s or doctoral degree from an accredited school of social work.
• Has one year of social work experience in a healthcare setting.
• Maintains compliance with state and federal regulations for this position.
• Carries current CPR certification.

Pay

$82,000.00 - $87,000.00 salary
