Mobile Unit Advanced Practice Provider (NP/PA)

About the role

The Advanced Practice Mobile Provider delivers comprehensive primary care across the lifespan (birth through end of life) while primarily based out of our Goodyear health center. You’ll practice with a high level of autonomy in a mobile health unit, supporting agricultural workers, local schools, and community health events, while collaborating closely with an interdisciplinary care team to expand access for underserved communities.

Responsibilities

  • Provide high-quality, comprehensive primary care services to a diverse patient population in mobile and outreach settings.
  • Evaluate patients in a timely, courteous, and professional manner; complete documentation and billing-related requirements accurately and on time.
  • Educate patients on diagnoses, treatment plans, preventive care, and how to navigate the healthcare system.
  • Develop treatment plans consistent with clinical impressions and working diagnoses; coordinate referrals and collaborate with other disciplines involved in total patient care.
  • Ensure continuity of care through appropriate transfer of information and follow-up planning.
  • Promote excellent customer service for both external and internal customers; model Adelante’s values in every interaction.
  • Support Quality/Risk initiatives through participation in peer review, medical record audits, protocol review, and quality improvement activities.
  • Represent Adelante Healthcare in the community when requested and participate in outreach planning that addresses community health needs.
  • Rotate call coverage for emergencies in the evenings, weekends, and holidays as assigned.

Qualifications

  • Graduate of an approved Nurse Practitioner program or Physician Assistant program.
  • Active Arizona NP certificate (AZ Board of Nursing) or active Arizona PA certificate (AZ Regulatory Board of Physician Assistants).
  • Valid Arizona Advanced Practice license or Arizona Physician Assistant license.
  • Valid DEA registration.
  • Current BLS; CPR/AED certification for the healthcare professional (per AHA/Red Cross guidelines).
  • Valid Arizona driver’s license.
  • Ability to provide care across the lifespan (birth through end of life).
  • Strong procedural skills including suturing, toenail removal, staple removal, and foreign body removal.
  • Demonstrated cultural competency and ability to work effectively with people of varied backgrounds and social circumstances.
  • Ability to obtain/maintain a Level One Fingerprint Clearance Card (as required for specialty behavioral health locations).
