MMTP Counselor - Methodist Clinic - Full Time/Days
About the role
The counselor will provide supportive services to patients undergoing treatment for substance abuse, focusing on personal, social, and vocational rehabilitation.
Responsibilities
- Screen and interview new patients, review records, and confer with other professionals to evaluate their substance use history, mental and physical conditions, and suitability for treatment.
- Conduct chemical dependency program orientation sessions.
- Counsel patients individually or in group sessions to help them overcome dependencies, adjust to life, and work towards treatment goals.
- Review and evaluate patients' progress against set goals in treatment care plans.
- Facilitate patient groups and document sessions in accordance with APG guidelines.
- Gather psychosocial history and updates every five years.
- Develop individualized recovery plans based on various concerns including chemical dependency, fiscal, mental health, medical, vocational, employment, and family issues.
- Maintain accurate patient records in compliance with OASAS, NYC-DOH-MH, CSAT regulations, and Joint Commission standards.
- Document all patient sessions following APG guidelines.
- In collaboration with the interdisciplinary team, evaluate patient's medication management, treatment concerns, and recovery plan updates during multi-disciplinary team meetings (MDT).
- Complete other work assignments such as financial updates, HRA/STARS reporting, and family updates within required time frames.
- Provide educational, HIV, family, vocational, and fiscal treatment services, information, and referrals to patients.
- Maintain good community relations by participating in community tours to monitor patient compliance with Mount Sinai Beth Israel policies and procedures.
- Collect patient toxicology specimens as required by state law.
- Refer patients to appropriate agencies to ensure adequate care of their needs.
- Input patient data into HRA/STARS and PAS 44, 45, and 26 into OASAS data banks.
- Receive weekly supervision.
- Plan or implement follow-up or aftercare programs for clients discharged from treatment programs.
- Act as a backup counselor when a counselor is absent from work.
- Work Saturdays as required.
- Participate in the development of other staff members.
Qualifications
This position is union-represented under SEIU 1199-MSBI. It requires a strong commitment to fostering a respectful and inclusive workplace, and the ability to collaborate effectively with interdisciplinary teams. The ideal candidate should have a bachelor's degree in psychology, counseling, or a related field, along with relevant experience in substance abuse counseling and clinical settings. Excellent communication skills, empathy, and a compassionate approach are essential. Proficiency in computer software and databases is also required.
