Key Responsibilities
- Serve as the primary point of contact for military-affiliated clients and individuals with disabilities.
- Assess client needs and connect them with appropriate programs, benefits, and community resources.
- Aid clients with completing applications, forms, and required documentation.
- Cook up referrals to healthcare, employment, education, housing, financial assistance, and other support services.
- Maintain accurate and confidential client records and case documentation.
- Monitor client progress and provide follow-up support to ensure services are effective.
- Collaborate with internal departments and external organizations to coordinate care and resources.
- Educate clients about available benefits, accommodations, and support programs.
- Advocate for clients while ensuring compliance with applicable laws, regulations, and organizational policies.
- Prepare reports and maintain records of client services and program outcomes.
Qualifications
- A high school diploma or equivalent is required.
- An associate's or bachelor's degree in Social Work, Human Services, Psychology, Public Administration, or a related field is preferred.
- Experience working with veterans, military families, individuals with disabilities, or in human services is preferred.
- Knowledge of community resources and government assistance programs is an asset.
- Strong organizational, communication, and interpersonal skills are necessary.
- Proficiency with Microsoft Office and case management or client database systems is required.

Working Conditions
