Military and Disability Client Services Coordinator

Key Responsibilities

  • Serve as the primary point of contact for military-affiliated clients and individuals with disabilities.
  • Assess client needs and connect them with appropriate programs, benefits, and community resources.
  • Aid clients with completing applications, forms, and required documentation.
  • Cook up referrals to healthcare, employment, education, housing, financial assistance, and other support services.
  • Maintain accurate and confidential client records and case documentation.
  • Monitor client progress and provide follow-up support to ensure services are effective.
  • Collaborate with internal departments and external organizations to coordinate care and resources.
  • Educate clients about available benefits, accommodations, and support programs.
  • Advocate for clients while ensuring compliance with applicable laws, regulations, and organizational policies.
  • Prepare reports and maintain records of client services and program outcomes.

Qualifications

  • A high school diploma or equivalent is required.
  • An associate's or bachelor's degree in Social Work, Human Services, Psychology, Public Administration, or a related field is preferred.
  • Experience working with veterans, military families, individuals with disabilities, or in human services is preferred.
  • Knowledge of community resources and government assistance programs is an asset.
  • Strong organizational, communication, and interpersonal skills are necessary.
  • Proficiency with Microsoft Office and case management or client database systems is required.
