Responsibilities

  • Support consumers in achieving personal care goals and addressing ongoing challenges.
  • Educate consumers on medications, track logs, and conduct monthly audits.
  • Cook counselors coordinate services based on individual needs.
  • Deliver high-quality care in line with Individual Rehabilitation Plans (IRP).
  • Accurately document services within required timeframes using electronic health records.

Requirements

  • Education & Experience: Bachelor’s Degree in mental health, psychology, sociology, or social work, human services or a related field from an accredited college or university OR A combination of one (1) or more years’ of college, plus related life or work experience equal to four (4) years OR High school diploma or GED plus four (4) years of related work or life experience required.
  • Skills: Strong verbal and written communication skills, with proficiency in Microsoft Office Suite, including Outlook, Excel, and Word. This role requires driving as an essential function. Candidates must have a valid driver's license and be subject to post-offer verification of driving eligibility consistent with company insurance requirements.

Pay

  • Hiring Min Rate: 18 USD
  • Hiring Max Rate: 18.75 USD
