MG Pediatric Social Worker EX

SUMMARY
Responsible for providing social work services to patients and families. Performs in accordance with the facility’s policies and procedures. Follows the facility’s standards for ethical business conduct. Conducts self as a positive role model and team member. Participates in facility committees, meetings, in-services, and activities.
RESPONSIBILITIES
- Assesses patient and family psychosocial needs and develops plan of care in concert with physicians, nurses, and therapists.
- Links patient/family to appropriate community resources including information/referral to sources of financial assistance, transportation, food, housing, support groups.
- Provides short-term counseling to identify problems and facilitate needed referrals.
- Serves as patient advocate and liaison with physicians, families, and community to ensure continuity of care.
- Documents patient/family status and history, diagnosis, treatment plan, evaluation results, observations, and progress in medical record.
- Serves as a resource to other staff on community resources and in area of specialty.
EDUCATION
- Master's degree in social work. Additional course work in area of specialty (e.g., psychiatric, pediatric) as required.
EXPERIENCE
- Two years experience in social work with broad background with diverse patient populations, preferably in medical setting. Additional one year experience in area of specialty.
REQUIREMENTS
- Eligible for licensure if required by state or position.
KNOWLEDGE
- Knowledge of social work principles and practices. Stays abreast of state of art in field.
- Knowledge of how to use a variety of diagnostic tools and techniques including DSM IV-R.
- Knowledge of the development and maintenance of information/referral systems.
SKILLS
- Skill in assessment, diagnosis, treatment, counseling, and care management.
- Skill in proper use of tools such as the DSM IV-R for identifying clinical symptoms.
- Skill in building and maintaining community resource database by establishing effective liaisons with key sources of assistance for patients and families.
ABILITIES
- Ability to interact effectively as a member of an interdisciplinary health care team.
- Ability to use community resource database computer software effectively.
- Ability to communicate appropriately with diverse patient populations and conduct effective community outreach activities.
WORKING CONDITIONS
While performing the duties of this job, the employee is frequently required to walk; stand; reach with hands and arms; climb or balance; stoop, kneel, crouch, or crawl; and talk or hear. The employee is occasionally required to sit or stand. May be exposed to various patient conditions including infectious diseases, blood, and bodily fluids.
