About the role
This position aims to prevent mental health issues from escalating by connecting families early and promptly with necessary support. The goal is to alleviate the long-term impacts of traumatic experiences and enhance the behavioral health of children within the service area.
Responsibilities
- Conduct behavioral health and social needs screenings on all families
- Provide resource options to families
- Follow up with families and communicate outcomes with referral sources
- Work with families to identify barriers to obtaining behavioral health treatment and social needs resources, and problem-solve alternative solutions
- Collaborate with crisis center staff on children in crisis
- Screen families for social needs such as transportation, food, home safety, utilities, medical, benefit needs, supplies, child care activities, and job training
- Link families to appropriate resources based on identified needs and ensure they have access to these resources
Requirements
A Bachelor’s Degree in Social Work or a related field is required. One to two years of experience in Social Work or Counseling is preferred.
