Mental Health Practitioner - Housing Navigation

Duties and Responsibilities

• Provide instruction and monitoring on the identified service areas indicated on the client's Housing Focused Person Centered Plan, other assessments, and other recommendations by client's treatment team.
• Engage in completing some Housing Focused Person Centered Plans with people referred.
• Continually monitor progress and modify the Housing Support Plan and Housing Support Crisis Plan as appropriate.
• Communicates changes in service needs and recommendations to Clinical Supervisor/Mental Health Professional, Case Manager, and rest of treatment team on an ongoing basis to coordinate care.
• Maintain records of progress for each face-to-face meeting and telephone contact with client. Documentation will include an assessment of client's ability to function and manage in their own apartment.
• Aid clients to create a plan to work towards service goals in regular meetings with the client.
• Schedule appointments to see the person to assist in requested services for the amount of time and frequency agreed upon in the person's support plan.
• Bill for services by completing contemporaneous documentation as required by program policies and procedures.
• Intervene as necessary for emergencies and/or when he/she assesses that the person may need when in crisis. Connect the person to appropriate emergency services, doctors, staff, and emergency family contacts when necessary.
• Cookordinate and cooperate with other agency and community providers to provide the best possible care for the individual, including but not limited to landlords and other community resources.
• Complete all necessary Release of Information and documents for communication with other providers and keep these current for each person served.
• Aid person served in obtaining and maintaining benefits including housing subsidies and other economic assistance, as well as providing assistance in budgeting monthly funds to maintain housing.

Requirements

• Candidate will have a Bachelor's degree in behavioral health sciences plus 2,000 hours work experience providing proving supportive services to vulnerable populations (people experiencing homelessness, unstable housing and/or people with co-occurring substance use disorders and mental illnesses)-OR-No college degree and 4,000 hours supervised work experience providing proving supportive services to vulnerable populations (people experiencing homelessness, unstable housing and/or people with co-occurring substance use disorders and mental illnesses).
• Must have car, auto insurance and driver's license.
