Mental Health Practitioner- FACT NORTH

About the role

New Horizons of the Treasure Coast and Okeechobee is a comprehensive mental health and substance use recovery agency serving 14,000 clients annually across four counties. The position is based at the FACT north office.

Responsibilities

• Develop, write, implement, evaluate, and revise recovery goals and plans in collaboration with the ITT (Individual Treatment Team)
• Make appropriate changes to treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior
• Provide individual vocational-supportive counseling to help members identify strengths, set goals, and address mental illness symptoms that affect work
• Offer individual supportive therapy, social-skill development, and assertiveness training to enhance community involvement
• Prepare written documentation for each service provided
• Perform other duties as requested

Requirements

• Master’s degree in a behavioral science, Social Work, or Counseling
• Experience working with adults with severe and persistent mental illnesses or similar human-services needs
• Pass Level 2 Background check through DCF and AHCA Clearinghouse
• Valid Florida Driver's License with 6 points or less in the last 3 years
